CPDS facilities are conveniently located to receive referrals from dental offices (general and pediatric) not equipped or choose not to perform IV Sedation Dentistry. Our referring dentists can be rest assured of not loosing patients to “a competitor” because CPDS only specializes in IV Sedation Dentistry. Simply put, we guarantee a specialty service that is complementary and not competitive to your office.

SEDATION CANDIDATES

Our referring dental offices typically refer pediatric patients who are (have):

  • Extremely Anxious or Fearful
  • Behaviorally Challenged
  • Medically Challenged
  • Failed Other forms of Dental Treatment (i.e. nitrous and oral sedation)
  • Complex & Extensive Treatment Plans
  • Desiring to Treat in One visit
  • Parental Preference for IV Sedation

REFERRAL PROCESS

Our referral and treatment process is simple as:

  1. Calling CPDS Dental Hotline 1-844-68C4PDS (1-844-682-4737 ) if you desire to consult with a CPDS dentist
  2. Complete CPDS Referral Form
  3. Attach most Recent Imaging (if available)
  4. Fax or Email form to CPDS  Fax: 404-521-4044 ◊ Email: [email protected]
  5. Leaving the rest of care coordination to CPDS (contacting and scheduling patient)
